NOMINATION FORM

Deadline for submission of 2024 nominations is Friday, May 10^th.  The application with original signature is to be received in the Lima Allen County Chamber of Commerce office by 4:00 PM on the deadline date.  The Lima/Allen County Chamber of Commerce office is located at 144 S. Main St., Lima, OH  45801. Online nominations are also due by 4:00 PM on Friday, May 10.

The Chamber will honor the 2024 inductees to the Allen County Agriculture Hall of Fame at a banquet to be hosted July 25^th at the Lima Civic Center.
